The timer begins as soon as Sonarr detects an episode has a release available. This release will show up in your Queue with a clock icon to indicate that it is under a delay. It retries failed episodes (different versions, not the same one), and will blacklist bad releases so it doesn't retry them. Sonarr will also remove entries in your download client (utorrent, deluge, sabnzbd, nzbget, etc) so that it cleans up the history for you.
